For the nearly 30,000 people who call Mfangano Island home, access to emergency medical care can depend on one critical journey across Lake Victoria.
While the island's public, private, and faith-based health facilities provide essential care close to home, patients experiencing life-threatening emergencies often need specialized treatment available only at mainland hospitals. Before the Mfangano Emergency Boat, families frequently faced dangerous delays as they searched for transportation, often relying on public ferries or costly private boats during moments when every minute mattered.
Since 2013, the Mfangano Emergency Boat has provided the island's only 24-hour emergency referral service, ensuring that patients can reach higher levels of care quickly and safely. Supporting every major healthcare provider on the island, the program has coordinated more than 400 emergency transfers and continues to strengthen access to healthcare for communities across Mfangano.
A community-led solution with lasting impact
The Mfangano Emergency Boat reflects what is possible when communities, local health providers, and partners work together to remove barriers to care.
More than 90% of emergency referrals involve mothers experiencing pregnancy or childbirth complications, making the program an essential part of improving maternal and newborn health on the island. Over the past four years, every obstetric patient transported through the program has reached definitive care with a positive outcome. Independent research has also shown that the Emergency Boat significantly reduces delays in accessing emergency care, helping patients reach treatment in hours rather than days.
Beyond emergency transport, the program brings together Ministry of Health facilities, private and faith-based clinics, Beach Management Units, community health workers, and local leaders to strengthen emergency referrals, provide community health education through local radio, and coordinate ongoing improvements to the island's healthcare system. The boat itself is built and maintained locally using traditional boatbuilding techniques, creating a sustainable solution rooted in community knowledge and leadership.
